Since I've been ad AE at this company for almost 26 years and I have clients that I've had on the air from the time I started to present day, I don't give out as many cards as other newer salespeople. I think I average 1 or 2 a week.
We cover two markets in Louisiana and I often ride with our salespeople, so I give mine out daily.
As often as I am asked. The QR code does two things: acts as a logo, and links to a page on my website for complete contact information and e-mail addresses that would take three cards to list.
I tend to hand out my cards most frequently at networking events, including BNI (Business Networking International) and Chamber Mixers. Also, I almost always leave a card even if the owner or person who handles marketing isn't available. Sometimes I write a personal note "Sorry I missed you" or "Have a great day" just to let them know I'm thinking about them as people and their businesses, and not just the business.

I give mine out as often and whenever I can, just like candy. They don't do me any good in my wallet, biz card holder or in the car. The more "touches" I get with someone, the better...prospects, new or existing clients. BTW...the printer goofed and forgot to put my email on the card, so I printed small address labels with my email and put it on the back of the card. Didn't want to waste them.
Having new ones made this week; will post the new design when I get them.
